Either of these 2-page forms must be completed in full and signed by a parent in two places on the second page. - Physician's Report Form
This 2-page form must be signed by your child's physician. A physical is not required; only confirmation that your child is developmentally healthy enough to benefit from participation in a group child care setting. - Student Participant Voluntary Excursion/Field Trip Waiver and Medical Authorization Form
This form must be signed by a parent for each child enrolled in the pre-school program, and must include contact, medical, and medication information. - Parent Participant Voluntary Excursion/Field Trip Waiver and Medical Authorization Form
This form must include contact, medical, and medication information. - Private Vehicle Safety Certification & Auto Service Record Forms
The first of these forms must be completed and signed by a parent who wishes to be able to transport Bay School children on field trips, while the second form must be completed and signed by an automobile mechanic within 90 days of application attesting to the safety of a parent's vehicle(s) which will be used on field trips. - Non School-Sponsored Trip Waiver/Voluntary Excursion for Staff and Volunteers
