SCHOLARSHIP APPLICATION AND INFORMATION

DESCRIPTION

 

This award, for academic achievement, is open to all graduated or graduating high school students that have not yet attended a college or university. It provides a minimum scholarship award of $500 to be used to offset expenses related to the completion of an associates certificate or baccalaureate degree at any accredited college or university. This award is made directly to the award recipient.

 

ELIGIBILITY

 

American citizens of Italian descent that have applied to and have been accepted to attend an accredited college or university.

 

OVERVIEW

 

This award is not based on financial need, nor athletics. It is an award for academic excellence and considers high school academic achievements, overall GPA, SAT and ACT scores.

High school seniors, or high school graduates not yet attending a college or university, must

submit with their application:

 

A sealed envelope containing an official transcript of all secondary school courses
of study; academic grades; identification of advanced placement studies; honors
classes, GPA, class rank, and verification of SAT/ACT scores.

 

The official transcript should include appropriate signatures by school officials, and
the school seal. Copies or computer print-outs are not acceptable unless they
include appropriate signatures and seals.

 

Letter of college/university acceptance must be included with your submission.

Piecemeal submissions will be immediately rejected.

 

GENERAL

 

All applicants, excluding active lodge members in good standing and their immediate family,
defined as husband, wife, children and grand children only, must be a legal resident in the state of Georgia at the time of application.

 

Members of the OSIA Marietta Lodge # 2607 and their immediate family as defined above, will receive first consideration if the active member has been an active member in good standing for the past two years.

 

Failure to comply with all requirements including timely submission, cross outs, spelling errors, distortion of any facts will be cause for rejection of the application.

The application must be a professional representation of the individual.

 

Essay questions should be approached and answered as one would a college level paper with meaningful substance which responds to the specific questions asked.

 

When completing the application the applicant must either type or print in block print using black ink. Do not use blue ink. Use additional pages as necessary.

 

Application and all supporting documentation must be received together no later than May 1st of the calendar year. Completed application with all supporting documentation should be mailed to:

Only the individual selected to receive this award will be notifed. Notification will occur During

the month of June if not sooner, for the college year beginning the following September.

 

Applicants in submitting an application agree to waive all rights of protest, or to view or discuss the decision and results of the scholarship committee recommendations with committee members, and/or elected lodge officers, or lodge members. The decision of the scholarship committee is final.
