• Emergency Contacts and Sign Out Authorization
• Adult Volunteer (21 or older) [Only the persons listed on this form will be allowed to pick up your child before car line.]
• Junior Volunteer (14 - 20 years old)
• Medical Form [This completed health form must be on file for every person attending day camp. No one may participate in camp if this form is not on file.]
• Tiger Cub [For boys attending 1st grade 2009-10]
• Youth Registration [For boys who will be Wolves, Bears, & Webelos in 2009-10]
• Gatorbite Registration [Non-scout registration for Volunteers’ Children]

Everyone who attends camp (scouts/volunteers/gatorbites) MUST have a medical form completed and submitted at time of registration. All packs must provide a 4:1 ratio of scouts to volunteers everyday. Remember that 2 part-time volunteers equals one full time! MWF + TTH = ONE full time (M-F)
Volunteers can register younger siblings for Gatorbites as long as they are potty-trained.

Day Camp rank is one rank higher than his current rank. Spring recruited tigers will be registered as Tigers.

IMPORTANT DATES:

March 19	Pack Registration Drop-off, 6:30pm @ FBHS
April 9	Last Day to Turn in Pack Registration, 6:30pm @ FBHS
May 9	Camp Volunteer Training to include Aquatics, Shooting Sports & Camp Specific Den Leader Training, 9 AM-12 PM @STUMC
May 23	Swim Test and T-shirt pick-up, 9 AM - 5 PM @ 5 Lakes
May 27	Swim Test and T-shirt pick-up, 3 PM - 6 PM @ 5 Lakes

1. How long is Day Camp?   Day Camp lasts for one week.
2. Which week should my son attend?   As a Pack, we will attend week 1, so you should make your best effort to attend that week. Your son can attend either or both weeks, however.
