![]() All of the following forms should be used by any chairperson who has an event which involves a budget. If you are the chair person for an event, please take the time to review the following forms: Copies of these forms can also be found in the PTO Mailbox in the school mailroom. Samples of how each form should be filled out is also in the mailbox if you need to see what information is being requested. If you need to request reimbursement or an advance, please use the Check Request form. If you need cash for an event: To turn in deposits, please complete and turn in the Deposit Notice with all funds.
At the end of the event, please be sure to complete both: If you are a teacher and would like to request the PTO to purchase something, please use the new Request Form |
