2. Complete order form including sizes, amounts,homeroom teacher and check payable to NDMS PTSO

3. Place form and money in an envelope labeled with student's name and homeroom teacher

4. Teacher will turn envelope in to the PTSO mailbox where the order will be retrieved and filled - and the merchandise will be delivered to your student in homeroom within 10 days.
