Turn your Box Tops in on this "Over The Summer!" Collection Sheet

Collect Box Tops to fill up sheets, circle the prizes that you'd like, then turn them in to your homeroom in September. Turn in as many sheets as you'd like, but only circle one prize per sheet! Each sheet (10 Box Tops) will go into a drawing to win one of these prizes. Box Tops must be turned in by FRIDAY, September 25, 2009 to be in the drawing:

*Trim Box Tops and peel the cardboard off the backs to make it easier to tape on the sheet

*Be sure to write your name and room number on the sheet to go into the drawing!
