FAMILY DIRECTED RESOURCES

The goal of Family Directed Resources is to support the family to increase their capacity to care for the eligible individual in the family home, support the individual so he/she may be more independent in the family home and community, and support the individual so he/she may develop skills. Family Directed Resources pays for services/needs that can not be covered through other means.

Family Directed Resources is made of three areas of support. These include Basic Support Services, Therapy Training grant and Adaptation/Modification Grant.

BASIC SUPPORT SERVICES - Basic Support Services provides the opportunity for a wide variety of supports. These include the following:

Respite provides temporary care of eligible family member or provides the family with temporary relief from the care required to maintain the individual in the home.

Examples *Respite care in or out of the familys home
*Residential/Day/Weekend camp
*Group recreation activities
*Emergency respite care
*Other unique respite needs of the family
Purchase or Lease of Adaptive Equipment Ð Requires Professional Recommendation & not covered by health insurance.

Examples- *Wheelchairs or other ambulating devices

*Adaptive seating and supports
*Special communication devices
*Feeding and food preparation equipment
*Modified clothing
*Adaptive or specialized switches
*Specialized bathing and toileting devices
*Other equipment that meets unique needs of family

Home Modifications

Examples - *Inside and outside ramps and walkways

*Special railings
*Bathroom modifications
*Special fire alarm equipment
*Other modifications that meet unique needs of family

Special Diets Requires Professional Recommendation & not covered by health insurance.

Examples - *Commercial dietary supplements
*Nutrition enhancers
*Ingredients to supply the eligible individual with specific dietary needs
NOTE: does not include items for food allergies (i.e. soy milk in lieu of dairy milk due to dairy allergy)

Counseling, Training and Education for Family

Examples - *Individual or group counseling for family members

*General legal or financial training related to the care of eligible family members but not specific legal or financial services
*Registration fees for conferences are eligible at 100% cost. Hotel room is eligible at 50% of the total cost. (Does nt include food, phone charges, movie rental etc) Travel expenses are not eligible for reimbursement.

Summer Programs Integrated educational and/or therapeutic activities during the summer school break Criteria (all must be met)

*The request provides educational and/or therapeutic activities which address a documented delay
*The request service dates take plae during the summer months (June, July, August)
*The family is making other contributions such as transportation, registration fees, deposits, etc.
*The request is not a required school activity or program
*The request for seasonal program is received prior to July 15th
Examples - *Day camp
*Community classes or memberships which provide the opportunity to address a documented delay (i.e. gymnastics or pool pass for a child who has a documented OT/PT need)
NOTE: community classes and memberships: FDR may be used for the individual plus one caregiver/parent.

Other Requests which do not meet the criteria outlined under any other category of service but meet the unique needs of the family

Criteria - *The request is inline with the goals of FDR
*The request is related to the eligible individuals documented delay or developmental disability
*The request is not a typical expense incurred by a family
Examples - *Diapers for a child beyond training years (3+yrs) when not covered by insurance.
*Adult day programming/care
*Training or programming to aid the individual in developing skills for transition
*Other unique needs of the family or individual
*Non-adapted therapeutic/educational materials used as an aid for the family to provide extra therapeutic or educational opportunities for the eligible individual (limited to $200.00 per year)

ADAPTATION/MODIFICATION GRANT Ð includes Transportation Grant/Fencing - Provides funding for tangible non-typical items that remove barriers for individuals with developmental disabilities in their home and community.

Criteria - *The request is a one-time non-recurring purchase within a program/calendar year
*The request is not a typical expense incurred by a family
*The request is a tangible item
*The request is for a modified or adapted piece of equipment
*Other sources of funding have been investigated and verified with a denial

Transportation Grants Ð Provides funding to assist individuals and families to meet transportation needs as an alternative to the Boards traditional transportation services

Fencing Provides funding for families for fencing of the family home

THERAPY TRAINING GRANT Provides funding to train families and other natural supports in order to give them the tools they need to support the eligible individual with ongoing therapeutic/behavioral needs. May also include materials and equipment needed to carry out skills learned during training

Criteria for training requests (all must be met)
*The family has not accessed Basic Support Services for the same therapy within the same program/calendar year
*The request is not a typical expense incurred by a family
*The request is for training, assessment and/or consultation for the family.
*The request is directly related to a documented behavioral/therapeutic need/delay
*Other sources of funding have been investigated and verified with a denial
Criteria for material and equipment requests (all must be met)
*The request is not a typical expense incurred by a family
*The request is for materials and/or equipment necessary in order to carry out a current identified therapeutic/behavioral goal
*The request does not duplicate the purpose of another request

REQUESTING A SERVICE

*Family completes, signs and sends the appropriate request form to the FDR Coordinator
*Coordinator reviews and approves/denies request
-If approved, Coordinator processes request
-If denied, Coordinator notifies the family
*Original invoices, quotes, receipts are required in order to process payments. Faxes and copies will not be accepted.
